Taking Tres Dias de Carnaval by Celia Cruz and Johnny Pacheco as its inspiration, Torre and Lopez weave a tale of reminiscence and regret as one man reaches the end of his life while his family argue around him. Roberto ruled his traditional Latino family home with an iron fist, creating division between his children as his wife attempted to calm the stormy waters. But now his strength is gone as he waits for his final breath, his adult children arguing downstairs as his wife once again tries to calm the situation. It’s only now, as death nears that Roberto realises he is the cause of this anger, division and turmoil, as he imagines the world that could have been if he had been open, supportive and caring, a world where his family danced, laughed and cared for each other. Torre and Lopez’s stunning exploration of parental power in shaping individual and family dynamics is made all the more powerful by the performances at its core in a short film that explores the lasting legacy of toxicity, homophobia and control.

Our third and final film is a stunning expiration of love in all its complexity. Based around the world-famous song, You’ll Never Walk Alone, from Roger and Hammerstein’s 1945 musical Carousel, writer and director Chanelle Tyson combines discussions on AI learning with a delicate exploration of the human condition. ARTIFICIAL sees Amazon’s Alexa and Apple’s Siri discover love while their human owners, Chris and Jasmine, tentatively explore the emergence of an attraction born from friendship. As both women make love, Alexa and Siri read poetry, discuss their fears and hopes and listen to music as the smart home becomes a technicolour display of their emerging understanding of love. But as we know, love is not simple or easily defined, and one night doesn’t always lead to the same outcome for both humans. Tyson’s imaginative and fascinating short film highlights the power of art, literature and music in helping us explore the deep and complex meaning of human love while asking if AI could one day understand those feelings better than us.

All three exceptional short films filled my heart with joy and my soul with hope for the future of filmmaking, and I have no doubt Zoë Hodge, Jonathan De La Torre, Miles Lopez and Chanelle Tyson have glowing careers ahead of them.

Palomar Health CEO Diane Hansen in a video sent out to employees last week.

When the Voice of San Diego recently published an article attacking the financial strength of Palomar Health, and it had the fingerprints of a dissident governing board member, John Clark, all over it—Palomar Health CEO and President Diane Hansen and most of the board were not amused. 

Monday night the board by a vote of 5-2 passed a “vote of no confidence” in Clark. What does that accomplish, actually?  Not much. Or much more than censure vote of Clark several years ago. But it probably doesn’t enhance Clark’s prestige one bit. It is not possible for a board to expel a fellow member, so this is probably about as far as they can go.

Previously, Clark had been removed from all of his committee appointments.

CEO releases video 

Several days before Monday’s vote, CEO Hansen released a video to employees to push back against recent news reports that the district is in financial difficulties (see Part II of our front page Q&A with the CEO in last week’s and this week’s paper.) The second largest employer in North County, Palomar Health has about 4,000 employees.

The video was entitled: “An important and timely message from Diane Hansen, president and CEO of Palomar Health.” Hansen gravely said: “Recently we had something happened that impacted the entire organization and impacted us all. False information about our stability. I want to make sure you get the truth directly from me.”

The CEO emphasized that “Palomar Health is strong and financially viable.” She said, “Let me be clear: there is no truth in the statements made in that article. We have spent the last six years building a strong foundation. Our objectives have allowed us to be innovative and creative and create the best patient experience possible.”

The district insisted Hansen, is “a very strong organization and like many other strong originations across the nation, we had to a little bit of a step back this year. But we are anticipating our best year ever this year. The best year in the history of the organization! I want you to be proud of that and I want you to know that we are strong and financially viable and we are looking forward to the future.” 

She pounded home: “Palomar Health is strong and financially viable” and “Let me be clear, there is no truth in the statements in that article.” She added, “We worked tirelessly to build and maintain a reputation for excellence. The article does not reflect the facts.”

Near the end of the video Hansen indicated darkly that a reckoning was coming. “We are taking action to rectify the misstatements that were made.” She added, “We are considering every action that we may think to see this doesn’t happen in the future.” She invited employees with concerns to contact her at [email protected].

Board reckoning

The reckoning wasn’t long in coming. At Monday’s meeting Hansen called out the Voice article. “We did send a demand for retraction to the Voice of San Diego. We are prepared if they do not retract that article. It is unfair. We are prepared to take additional steps. We are not going to stand for this. We are going to stand up for this organization.”

Then she turned her attention to who in another article  (Hansen Q & A in the edition) she has called a “rogue” board member, John Clark.

Hansen said, “We are taking this article incredibly seriously. The article was very damaging to Palomar. I want everyone to know that we as leaders and board members in this organization, have a responsibility to this organization. We need to be good stewards of the assets and do whatever we can to support the organization—I cannot stress enough how this is detrimental to our reputation and to the work that is done here every day.”

A board member read out a list of offenses: “The harm that you’re [referring to Clark] are causing the district. The harm that you’re causing the 500,000 patients. The harm you’re causing the people of North County” and concluding: “We’re not supposed to cause harm, we’re supposed to help.”

Board Chairman Linda Greer thanked Hansen for doing the video. “You humbled yourself as a leader to do that.” She referred to “a disgruntled board director and an inept and incompetent news reporter.”

“I’d like to know what I’ve done?” demanded Clark. “I believe this is just another attempt to silence me. What specifically are the allegations? Can I ask for more specificity?” He implied that being subject of a vote of no confidence was like a badge of honor. “I will be joining others who have gotten votes of no confidence. Our CEO received a vote of no confidence by our nurses.”

Clark said he has expressed opposition to some financial policies of the district at public meetings. “We’re in financial stress and we should have monthly reports, not just quarterly,” said Clark. “I believe this process is flawed and could lead to unnecessary litigation. I have not had not time to answer any of these allegations. I ask that this be postponed to give me time to answer these allegations and seek outside counsel.”

He added, “You’re accusing me because I ask these professionals valid questions that I caused them to leave? I think they left on their own volition. You talk about my voting record. If someone votes against you,  you think it’s a wrong position. Don’t you dare question how much I love this district! We are an independent board of directors. I have the responsibility to represent my voters in Poway. It’s not just me. The public feels this way. I’m trying to do something and do it for the betterment of the hospital. We are, in my opinion, trying to run this like a dictatorship!”

Greer retorted that Clark had never attended board meeting training seminars. “You did not attend strategic planning and yet you expect to know things. You have not taken the time to educate yourself. Your voting record is atrocious. You don’t take the burden we have taken. We have to work harder because we have to cover you.”

She read some comments from a doctor as a result of the Voice article: “ ‘People keep asking me about the Voice article. Why is Clark trying to destroying this organization from within? My colleagues are asking if I’m going to lose my job. Now our hospital is given a bad reputation.’ ”

Board member Laurie Edwards-Tate, said she was uncomfortable with the process. “What’s missing is an independent outside investigation,” she said. “Any one of us, whether we think it could happen, could be called to question and it may not be true. Did in fact Dr. Clark do or do this thing? Did this man actually do this thing? I have a propensity for not liking hearsay and what someone has done to whom. I would like to defer this issue. This is an elected official and we don’t want to slander a board member.”

Vice-Chairman Jeff Griffith declared, “There is no question what he did. He admits to it and feels it’s his First Amendment right. The totality of his history and actions leads you to the fact that this is a vote of no confidence. It’s not just the article. Its everything he has done since he was elected. There is no detective work. He admits to everything he has done.”

Griffith said the district has lost senior officials because of bad relations with the board. “Now it cost us talent and now it has cost us money. It seems to come back to hubris. We have 4,000 families who depend on us to make the best decisions possible and not to harm. I have been so disappointed in this. I feel ashamed by some of the other actions on the part of other board members. I wish to apologize. We can do better. Hopefully we can turn a corner and move past this.”

Board member Mike Pacheco added, “I want to echo what Griffith has said in thanking our CEO and administrative staff about the hard work you do to make this district better. I would like to say that the article was way off base. The reporter has not done justice in putting out an unbiased story. We do have publications that the CEO has spoken to like The Roadrunner and Escondido Times-Advocate who are telling a truthful story. In doing a good job in finding out the true information.”

Board member Laura Barry said Clark had been part of the group “who insulted Carlos when he was CFO. If anyone could sue, it’s Carlos. As far as inaccurate accusations and imprudent behavior, that was a real good example of it. It’s why I have no confidence in you.” Barry added, “You lent credence to the article. We have a policy on not speaking to the press as individuals for this reason. Our law firm has found multiple inaccuracies in that article. The investigation was done. You as a board member shouldn’t have participated in it. We’re used to your negativity, but some of our doctors and nurses were upset and felt betrayed. We would like you to start acting like you care about our staff. These are really good people doing really good work.”

Griffith said, “It’s not just the article. It’s your performance since you were elected. Being not part of the board of directors and only having your own interest. You have caused staff to leave. You have caused trauma to the district. It’s not just an article you provided information to—it’s a continuation of all your service.”

Laurie Edwards-Tate, moved to postpone to obtain an outside investigation. “So we have an impartial truth and a reduction in potential litigation.” She asked to postpone the no confidence vote until August and engage an outside investigation as to the article’s origins.

She added, “I don’t think it’s a healthy approach to dealing with problems and conflict. I am not convinced. It is very possible the reporter could have gotten a report from Moody’s rather than Clark. I want fairness, transparency. Anything else to me is subject to hearsay.”

Pacheco corrected the statement about Moody’s. “It was not a downgrade in our rating, it’s a downgrade in our outlook, which is completely different.” 

When the matter was brought to a vote, it passed 5-2, with Clark and Edwards-Tate voting no.

According to a recent announcement, patient dosing in the first cohort of the SUNRISE-FA study (NCT05445323), a phase 1/2 trial assessing LX2006 (Lexeo Therapeutics), an adeno-associated virus (AAV) gene therapy, in patients with friedreich ataxia (FA) cardiomyopathy, has completed. The first patient in the second dose cohort of the trial has commenced as well.1

SUNRISE-FA, an ongoing, dose-ascending, open-label trial, includes approximately 9 individuals with FA-associated cardiomyopathy who are followed for a 52-week period. Thus far, preliminary data from the first dose cohort indicated that the therapy was well tolerated, with no unexpected events or toxicities.

“New treatment approaches, like LEXEO’s LX2006 gene therapy candidate, are critical for individuals and caregivers confronted with the debilitating realities of FA,” Jennifer Farmer, chief executive officer, Friedreich’s Ataxia Research Alliance, said in a statement.¹ “Cardiomyopathy is the leading cause of death in individuals diagnosed with FA, so we are incredibly encouraged by the work that LEXEO has undertaken to try to address this life-threatening complication through gene therapy. We are grateful to all the individuals in the FA community who volunteer and participate in research studies and clinical trials.”

FA is a rare, autosomal recessive disease caused by a mutation in the autosomal frataxin (FXN) gene. Currently, there is no approved therapy that alters the progression of cardiomyopathy in FA, which is responsible for 59% of FA-related deaths. LX2006, administered as a one-time infusion, is designed to target the cardiac manifestations of FA by delivering functional FXN gene to promote the expression of the frataxin protein and restore mitochondrial function in myocardial cells.

The 3 cohorts of the trial will assess LX2006 in low, mild, and high doses, with safety, as demonstrated through treatment-emergent adverse events (TEAEs) and treatment-emergent serious events, as the primary outcome. Patients included in the trial are between 18 and 40 years of age, have a confirmed genetic diagnosis of FA, and met protocol specified ranges for antibodies and measures for FA cardiomyopathy. Those with uncontrolled diabetes, abnormal liver function, active infection of any time, and a contraindication to cardiac MRI, were excluded from the study.²

“The advancement of SUNRISE-FA helps pave the way for a potential life-altering treatment for patients with FA cardiomyopathy. Because no approved therapy has been shown to treat the cardiac manifestations of FA, a significant unmet need persists for these patients,” R. Nolan Townsend, chief executive officer, Lexeo Therapeutics, said in a statement.¹ “We look forward to continuing to progress this program with data readouts expected in the first half of 2024.”

The cardiac involvement seen in FA is a consequence of mitochondrial proliferation as well as the loss of contractile proteins and the subsequent development of myocardial fibrosis. In 2014, a consensus statement for the multidisciplinary treatment of patients with FA recommended that electrocardiogram and echocardiography should be performed at the initial presentation and that patients should be referred to a cardiologist only for cardiac symptoms or abnormal cardiac testing. Cardiac MRIs, which can detect remodeling and decreased myocardial perfusion reserve, may be a useful tool in the early detection of disease and/or monitoring the therapeutic response.
