Assessing potential allergic reactions in COVID-19 vaccinated patients

The coronavirus disease 2019 (COVID-19) pandemic, which is caused by severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), has claimed more than 5.8 million lives worldwide. Several COVID-19 vaccines have received emergency use authorization (EUA) from regulatory agencies around the world, which has allowed almost 62% of the global population to be vaccinated against SARS-CoV-2.

Previous studies have revealed that vaccination has significantly reduced mortality, severe infection, and even transmission of SARS-CoV-2. Some of the COVID-19 vaccines that received EUA are Comirnaty® (BioNTech/Pfizer), Vaxzevria® (AstraZeneca), and Spikevax® (Moderna) vaccines.

The pace of COVID-19 vaccination has suffered as a result of vaccine hesitancy and vaccine supply shortages. Vaccine hesitancy is driven by various reasons, such as fear of possible allergic reactions and negationism.

Some of the adverse effects following COVID-19 vaccination have occurred due to the activation of protective immune responses; however, these are not allergic reactions. Immediate hypersensitivity reactions have been attributed to the excipients polyethylene glycol (PEG) and/or polysorbate 80 (PS80), both of which are used in most vaccines.

It must be noted that the reactions to these compounds are very rare. Diagnoses of hypersensitivity to these polymers remains difficult, as skin tests are not yet standardized and their efficacy is not well established.

Previous studies have shown that cases of immediate or delayed hypersensitivity to SARS-CoV-2 vaccines are very rare. Furthermore, the etiopathogenic mechanism of allergic reactions to these vaccines remains unclear. Research has shown that anaphylaxis with COVID-19 vaccines is around 7.9 cases per million doses worldwide.

A new study that is currently posted to the Research Square* preprint server while under consideration for publication in BMC One Health Outlook, analyzes suspected reactions in patients vaccinated with the first and/or second dose of SARS-CoV-2 vaccines. Herein, the researchers also explored the cause of these reactions among their components and evaluated whether such reactions interfered with the vaccination protocol.

About the study

In the current study, researchers conducted a cross-sectional and descriptive study on patients who were suspected to demonstrate hypersensitivity to SARS-CoV-2 vaccines. The participants were subjected to a skin prick test (SPT) and/or intradermal test (IDT) with the vaccines and their excipients.

Histopathological and immunohistochemical studies were performed by skin biopsy in cases where patients were positive IDT with a particular vaccine. Finally, scientists also performed basophil activation tests (BAT) and lymphoblastic transformation tests (LTT).

Study findings

A total of sixteen patients were suspected to show hypersensitivity to the SARS-CoV-2 vaccine. Of these, twelve patients received Comirnaty®, three received Vaxzevria®, and one received Spikevax®.

Eight patients had immediate hypersensitivity reactions, while the remaining showed delayed reactions. The SPTs to excipients and vaccines were negative in all cases. Further, IDTs with all excipients were negative.

In two patients who were selected positive IDT with the vaccine, histological and immunohistochemical studies showed T-lymphocyte involvement. In both cases, BAT and LTT were negative.

In 44% of the patients studied, the vaccination protocol could be completed. The remaining 56% did not receive the second dose, either because they refused to be vaccinated or because they already had the complete regimen.

Strengths and limitations

The researchers stated that the important clinical idea is the main strength of this study. The objective of analyzing the impact of an interventional screening program in high-risk patients is extremely important to guide public health policies.

However, the results should be interpreted with caution because of the small sample size and the fact that all participants came from a very specific region. To gain a better understanding of the issue, studies with larger sample sizes and greater representation of all possible regions are needed.

The vaccination protocol could be successfully completed in approximately half of the patients who demonstrated possible hypersensitivity reactions to SARS-CoV-2 vaccines. This is largely because of this allergological and immunohistochemical study and is, therefore, a significant achievement.

In the future, IDTs with vaccines could be a potentially valuable method for assessing the immunogenicity of vaccines.

New Casper City Council Members Take Oath of Office

The nonpartisan Casper City Council on Tuesday welcomed four new members and one re-elected incumbent who expressed their hopes for the next two years, and said farewell to four others who expressed their thanks for the opportunity to serve.

“My hope is that on council we may agree and disagree with one another as part of a robust productive decision-making process,” new member Amber Pollock said before Mayor Steve Freel administered the oath of office to her and the other council members.

“Second, is that we will be sensitive to the well-being and prosperity of our community and its citizens in all of our dialogue …,” Pollock said. “And last that we can work to practice to actively engage in the collective work of building and maintaining the community, a process that is never-ending and requirers civic engagement….”

Besides Pollock, the new council members sworn in were Lisa Engebretsen, Kyle Gamroth and Bruce Knell, who were elected in the Nov. 3 general election. Incumbent Steve Cathey won re-election and was sworn in, too.

Pollock and Knell represent Ward I in central Casper. The other Ward I representative is incumbent and outgoing Vice Mayor Khyrstyn Lutz.

Gamroth and Engebretsen represent Ward II in west Casper. They defeated incumbent council member Ken Bates. The other Ward II council member is Shawn Johnson.

Cathey represents Ward III in east Casper. The other Ward III council members are Freel and Vice Mayor Ray Pacheco.

The newly elected council members thanked the voters and their family members for their support.

Freel administered the oath of office to them, City Clerk Fleur Tremel administered the oath to Freel and Pacheco.

Before the oaths, outgoing council members Bates, Bob Hopkins, Mike Huber and Charlie Powell shared some experiences from their tenures, especially in the past year with the double-tap of the energy industry decline and COVID-19.

Bates was gracious in his departing words, and complimented the city staff and those on the boards he served. He learned an immense amount about the city and its functions that few know about.

Bates always dressed well for meetings because people dress well for their bosses who are the city’s residents, and he did everything he could to support Casper, he said. “Thank you so much for letting me serve you.”

Huber, like other outgoing council members, stressed the importance of the basic jobs of the city: police, fire, water, sewer, streets and other infrastructure.

He also chided those who might ridicule city employees as slackers, leaning on their shovels at job sites “We don’t have any shovel-leaners in Casper — it’s a work ethic from the bottom up and the top down.”

Hopkins, who joined the council in 2013, also complimented the city staff, saying they were as good as any employees in any corporation he’s known, he said.

Among the changes on the horizon, Hopkins said market-rate housing in the downtown area is coming. He added the city should consider helping Natrona County to build a new library.

Powell recounted a litany of changes in the past decade.

The city has replaced three fire stations by building new ones, built a new maintenance building, made significant changes to the long-neglected Hogadon Ski Area, performed earth work on the city-owned land used by the YMCA, supported the Boys and Girls Club, helped the Wyoming Medical Center with the new Masterson Place for families whose loved ones are in the hospital, expanded trails, built a new pool at Mike Cedar Park, upgraded facilities at the Casper Events Center, and supported the development of the David Street Station downtown.

The downturn in the energy industry and the resulting loss of tax revenues led to the city having to trim 20% of its work force and did that through attrition rather than layoffs.

The new council will need to grapple with how to pay for a new Fire Station No. 1, build a new police station, and upgrade the nearly 70-year-old Sam Hobbs Wastewater Treatment Plant, perform maintenance on its deteriorating streets, and continue with the Platte River Rehabilitation projects.

The outgoing council members didn’t mention some of the turbulent times regarding the fallout from former City Managers John Patterson and V.H. McDonald, nor the controversies over former Police Chief Jim Wetzel and former Fire-EMS Chief Kenneth King and their respective departments.

Powell said the new council members often will face decisions that must balance public safety with personal liberty. “You will receive a great deal of free advice.”

Like the other outgoing members, he said the city must focus on its basic infrastructure and services.

But the most important asset is the city staff because everything depends on them, Powell said.

Finally, he said he remains optimistic for the future.

“I believe Casper’s best days are ahead of us,” Powell said.

That’s what we’re exploring here. RWBY holds the souls of Ozma and Salem’s daughter’s and slowly as life gets worse and worse, they slowly become one.
How will Ozpin react to the host of one of his daughters dating his host? Or, even his children still being around? What about Salem? Will this change how the war goes or is history doomed to repeat it’s self?

‘Well-organised’ cocaine gang on Spain’s Costa Blanca is brought to justice – Olive Press News Spain

A Costa Blanca cocaine gang that processed and sold the drug has been busted by the Guardia Civil.

Nine Spaniards have been arrested including the gang leader at his Orihuela Costa home in the south of Alicante Province.

The Guardia Civil reported that the gang was ‘well-organised’, and that their leader was personally responsible for getting his hands on the pure cocaine.

Gang members then sold the drug on the Orihuela Costa and Pilar de la Horadada, as well as in Torre Pacheco in Murcia.

The operation was broken up by chance after a raid on a Pilar area farm where a greenhouse was being used to grow marijuana.

Inquiries uncovered that the marijuana production was a mere sideline to the main purpose of selling cocaine, as well as some ecstasy and hashish.

Once the gang boss got hold of the cocaine, his colleagues ‘cut’ the drug ready for distribution.

Then two of the leader’s closest confidants would then drive the supplies to their sales points.

The gang arrests happened over four days in December, with six of the detainees, including the ring-leader, remanded into custody pending their trial.

Guardia officers recovered 150 grams of rock cocaine, which had the potential to be sold as over 2,000 doses to customers.

5,000 marijuana plants were also seized along with guns, cars and €2,500 in cash which were some of the proceeds from the illegal sales.
