BOSTON – Ten individuals were arrested today for their alleged involvement in a drug trafficking organization (DTO) that distributed fentanyl throughout Southeastern Massachusetts and Rhode Island. During today’s arrests, 14.9 kilograms of suspected fentanyl and substantial amounts of cash and were seized from target locations.
According to the charging documents, in March 2021, law enforcement began investigating a DTO led by Estarlin Ortiz-Alcantara operating in Southeastern Massachusetts and Rhode Island. Intercepted communications on numerous cellphones allegedly identified Mario Rafael Dominguez-Ortiz, Yeury Francisco Garcia-Rodriguez, Rafael Cesar Cabreja Jimenez and Alfredo Rodriguez as members of the DTO who conspired with Ortiz-Alcantara to regularly distribute multi-kilograms of fentanyl from a base of operations at a stash house in Fall River. According to court documents, at the time of their arrest this morning, Cabreja Jimenez and Rodriguez had barricaded themselves in a back bedroom of the stash house upon the arrival of law enforcement.
The investigation also identified Rebecca Bartholomew, Edwin Collazo, Jason Cruz, Michael Pacheco and Jose Santiago as alleged regular DTO customers who re-distributed fentanyl to their own local customer bases in areas including Cape Cod and New Bedford. As part of the conspiracy, it is alleged that members of the DTO, under Ortiz-Alcantara’s management and control, conducted drug transactions with these regular customers in public areas – including in the diaper aisle of a store and in supermarket parking lots. It is also alleged that on at least one occasion, members of the DTO transported drugs concealed in cereal boxes while accompanied by a child. Over 500 grams of suspected fentanyl was seized during the investigation through controlled purchases and drug seizures.
The following defendants were arrested today and charged with conspiracy to distribute controlled substances:
Estarlin Ortiz-Alcantara, 36, of Providence, R.I.;
Mario Rafael Dominguez-Ortiz, 24, of Cranston, R.I.;
Yeury Francisco Garcia-Rodriguez, a/k/a “Benjamin Osorio Pizarro,” 32, of Providence, R.I.;
Rebecca Bartholomew, 36, of Yarmouth;
Jose Santiago, 45, of New Bedford;
Edwin Collazo, 26, of New Bedford;
Jason Cruz, 41, of New Bedford;
Michael Pacheco, 43, of Taunton;
Rafael Cesar Cabreja Jimenez; and
Alfredo Rodriguez.
“Today’s coordinated law enforcement operation disrupted an organization we believe served as a hub for the distribution of illegal and deadly narcotics in Southeastern Massachusetts and into Rhode Island,” said United States Attorney Rachael S. Rollins. “The opioid crisis has resulted in far too much harm and far too many deaths in our Commonwealth. My office will not relent in pursuing those who make a living off pumping poisonous fentanyl into our communities – violators will be identified, prosecuted, incarcerated and put out of business. I commend our federal, state and local law enforcement partners for their exceptional collaboration that led to today’s arrests and brought this case to fruition.”
“Opioid abuse is at epidemic levels in New England,” said Brian D. Boyle, Special Agent in Charge of the Drug Enforcement Administration, New England Field Division. “Illegal drug distribution ravages the very foundations of our families and communities here in Massachusetts and Rhode Island. Let these arrests serve as an example to those who distribute poisons like fentanyl. The DEA will aggressively pursue and hold you accountable. This investigation demonstrates the strength and continued commitment of our local, state and federal law enforcement partners.”
The charge of conspiracy to distribute controlled substances provides for a sentence of up to 20 years in prison, at least three years and up to a lifetime of supervised release and a fine of up to $1 million. Sentences are imposed by a federal district court judge based upon the U.S. Sentencing Guidelines and statutes which govern the determination of a sentence in a criminal case.
U.S. Attorney Rollins, DEA SAC Boyle, New Bedford Police Chief Paul Oliveira and Fairhaven Police Chief Michael J. Myers made the announcement today. Special assistance was provided by the Massachusetts State Police; Homeland Security Investigations; Bristol County Sherriff’s Office; and Fall River, Taunton, Attleboro, Scituate, Yarmouth, Providence (R.I.) and West Warwick (R.I.) Police Departments. Assistant U.S. Attorney John T. Mulcahy of Rollins’ Narcotics & Money Laundering Unit is prosecuting the case.
The details contained in the charging documents are allegations. The defendants are presumed innocent unless and until proven guilty beyond a reasonable doubt in a court of law.

Loulé is a traditional Portuguese market town in Central Algarve in Portugal. The small town is located a bit inland from the coast and is an excellent day trip destination from Faro and Albufeira.
Even though most visitors do not stay overnight in Loulé, there are still many things to do in Loulé Portugal. Some highlights include an Arabian-inspired covered market, the region’s best flea market, an ancient castle, many churches and handicraft stores around the old town.
In this post, I’ll show you how to get to Loulé from Albufeira and Faro and how you can spend half or a full day in Loulé Portugal.

What you need to know before visiting spending a day in Loulé Portugal
Before you start your Loulé day trip, take a look at some of these useful travel tips first:
The best day to visit Loulé is Saturday since both Loulé Saturday Market and the Gypsy Market are open on Saturdays only.
I recommend starting your Loulé day trip early. Try to arrive in Loulé by 9:00 am so you can comfortably follow my Loulé itinerary.
The best way to see Loulé is on foot as the town is quite small.
Use GoogleMaps app to navigate around Loulé.
Bring cash for purchases at the farmer’s market and Gypsy market.
Time your visit with these Loulé Festivities:
Carnivalof Loulé – the oldest carnival in Portugal (3 days at the end of February)
Festa de Mãe Soberana – a procession to honour Nossa Senhora da Piedade (late April)
How to get to Loulé Portugal
Even though Loulé is one of the most charming towns in Central Algarve, most visitors do not actually stay in Loulé. But rather, most visitors will visit Loulé from neighbouring towns like Albufeira or Faro.
Day trip from Albufeira to Loulé
There are 3 options to travel from Albufeira to Loulé: train, bus, and private minibus.
But to be honest, the train option from Albufeira to Loulé is too complicated: it involves a bus, a train then another bus and a lot of waiting time in between. So there really are only two good options.
Private minibus
The most direct wayto travel from Albufeira to Loulé is to join a half-day tour to Loulé. The tour is actually just a private minibus transfer from Albufeira to Loulé. And it is only available on Saturday.
The tour company will pick up passengers from hotels around Albufeira before 9 am and drop them off in Loulé Old Town at 9:30 am. Then at 12:30 pm, passengers are picked up at the same spot and returned to their hotels in Albufeira.
Public bus
If you take the bus from Albufeira to Loulé, catch the 8:00 am bus so you can have a proper day in Loulé.
Vamus Bus 10: from Terminal Rodoviário de Albufeira to Terminal Rodoviário Loulé
Time: 45 minutes
Cost: €4.55 one way, €9.10 return

Day trip from Faro to Loulé
Public bus
Vamus bus 10 travels from Albufeira to Faro and stops in Loulé on the way. This means that visitors from Faro can take the same bus in the reverse direction from Faro to Loulé.
Or the better option is to take Vamus bus 59, which travels from Faro to Loulé (and stops at Mar Shopping Algarve). Catch the 8:10 so you can be there before 9 am.
Vamus Bus 59: from Terminal Rodoviário Faro to Terminal Rodoviário Loulé

Best things to do in Loulé: day trip itinerary
There are many things to do in Loulé but you can see them all within half a day when you follow my efficient 1-day itinerary.
All you have to do is follow the numbered red pins on the interactive map and read the descriptions below.
Are you ready for your Loulé day trip? Let’s go!
1. See different food vendors at Loulé Saturday Market
The quiet town of Loulé comes alive on Saturday mornings when many food vendors set up their stalls around Mercado Municipal de Loulé. This farmer’s market only opens on Saturdays and is one of the biggest outdoor markets in the area.
I walked around and looked at each vendor and found many local specialties like bacalhau (cod), cheese, piri piri sauce, cured meats, bread, olive oil, honey and other Portuguese treats. And of course you can buy fresh vegetables, fruits, and flowers too.
People from all over Central Algarve go to Loulé to pick up the freshest produce every Saturday. It’s a local’s favourite! Maybe you can buy something too?

2. Visit the famous Mercado Municipal de Loulé
One of the best highlights of the Loulé day trip has to be Mercado Municipal de Loulé(Loulé Municipal Market), the largest covered market in southern Portugal.
The market was built in the early 20th century in a Moorish-inspired architecture that featured four pavilions with four gates. Many restoration were done throughout the years and in 2007, the market reopened with all the upgrades to reflect current hygiene and safety requirements.
When you visit the Loulé Municipal Market today, you can find many family-run stalls selling produce, fish, foods, locally sourced products and gifts around the perimeter. And there are several coffee shops and cafes where you can get a coffee and a snack.
This is considered to be the centrepiece of Loulé. You definitely need to spend a bit of time here.

Before you continue your Loulé day trip, have some food and coffee first.
I had a latté and a pasteis de nata at Bean 17 Coffee and more, which is located at Mercado Municipal de Loulé but it has its own entrance. I quite liked the cute coffee shop.
There is also another coffee shop inside the market called A Praça.
And if these cafes are super busy (because it does get pretty busy on Saturday mornings), there are several cafes on Rua Almeida Garrett. Check out L’Atelier Gourmet or O Postigo.

3, 4, & 5. Meander through the small streets of Loulé Old Town
After a good breakfast, roam around the narrow cobbled alleys around the market. It might feel like a maze because the streets go all over the place but that’s the fun part!
And many of these streets have small shops, cafes and restaurants, especially on Rua Almeida Garrett, which I previously mentioned.
Also look out for the Loulé Tunnel and Arco do Pinto(Pinto Arch). These are just some of the amazing things you will see in Loulé Old Town.

6. See Moorish architectural details at Igreja Matriz de São Clemente
After meandering the narrow streets of Loulé, you will arrive at Igreja Matriz de São Clemente (Church of San Clemente).
This 13th-century Gothic church has a plain whitewashed exterior and a simple Gothic portal. But what is interesting is that you can still see some of the Arab architecture in this church because it was built over an ancient mosque. Take a look at the bell tower – it was originally a minaret.

7. Wander around Antigo Convento do Espírito Santo
Antigo Convento de Espírito Santo (Convent of the Holy Spirit) was occupied by the Franciscan Friars of Nossa Senhora da Conceição and was a place for the female community at the end of the 17th century.
Since the earthquake of 1755, the complex was repaid and a neoclassical cloister was added. Then it was turned into a theatre and private residences.
Through all the repairs and conversions, the convent and the cloister remained. There is also a municipal art gallery and a 45m Araucaria (Norfolk Island Pine) tree in the middle of the cloister.

Address: Rua Vice Almirante Cândido dos Reis, Loulé | Hours: 9:30 am to 5:30 pm Tuesday to Friday; 9:30 am to 4:00 pm on Saturdays (closed Mondays and Sundays)
8. Look inside Ermida de Nossa Senhora da Conceição
I almost missed this small church until I saw a crowd at the entrance. That’s when I realized this is Ermida Nossa Senhora da Conceição(Chapel of Nossa Senhora da Conceição).
Built in the 17th century, the single-nave church has a very simple and clean facade. But when you go inside, there is an elaborate gold Baroque altarpiece with an image of Nossa Senhora da Conceição.
And adjacent to the gilded altar, the two walls are covered with blue-and-white azulejos tiles from floor to ceiling showing scenes of the Virgin Mary’s life. Wow! Just wow!

9. Visit Loulé Museum and Castelo de Loulé
First, check out the Roman and Bronze Age artifacts discovered in the Loulé region in the Loulé Museum. There are two rooms on the ground floor.
Then roam around Castelo de Loulé(Loulé Castle) and explore the upper floors and three towers. The castle itself went through several transformations as it was first built by the Romans, fortified by the Moors, and changed further when different rulers wanted to expand the castle.
However, Loulé Castle was badly damaged by the earthquake in 1755. What you see today is what was restored in the 1940s.

Address: Rua Dom Paio Correia, 17, Loulé | Hours: 9:00 am to 6:00 pm Monday to Friday; 9:00 am to 2:00 pm on Saturdays; closed Sundays | Admission: €1.62
10. Walk through Rua 5 de Outubro
Perhaps one of the most charming streets in Loulé is Rua 5 de Outubro (October 5th Street). It is the main pedestrian street in Loulé and this is where you can find cute boutique shops and alfresco cafes.
And take note of the pretty calçada Portuguesa (Portuguese pavement). This is a typical characteristic of a pedestrian street in Portugal where small pieces of stone are arranged in a pattern.
For now, walk through the street from east to west. You can come back later on to do a bit of shopping.

11. See the Baroque altarpiece at Igreja de São Francisco
Don’t just walk by Igreja de São Francisco (St. Francis Church), an 18th-century church with a pristine white exterior. There is a beautiful Baroque altarpiece in gilded carving made by the master carver, João Amado.
Unfortunately, the church was closed on the day I went. And I can’t find the hours of operations anywhere. So if you have a chance to look inside, let me know what you think.

12. See the latest cultural event at Convento de Santo António
Convento de Santo António(Convent of Santo Antonio) was built in the 16th century to replace a previous convent. But it closed after the church suffered damages from the earthquake of 1755. Part of the church is restored but the church has been closed since 1834.
In 1984, it became part of the Property of Public Interest and is currently used for different cultural events.
There isn’t too much inside the church except for an altar space where you can see the original interior walls. The rest of the church was transformed into an art gallery at the time of my visit (May 2022).
But I would still encourage you to peek inside the old church. It’s absolutely free!
Convento de Santo António
Address: Rua de Nossa Senhora da Piedade, Loulé | Hours: 9:00 am to 1:00 pm and 2:00 pm to 5:30 pm Tuesday to Friday; 9:30 am to 4:00 pm Saturdays (closed Mondays and Sundays)
13. Hike up to Santuário de Nossa Senhora da Piedade
Santuário de Nossa Senhora da Piedade (Sanctuary of Nossa Senhora da Piedade) consists of a simple chapel built in 1553 and a big dome church built later on in the 20th century.
But what’s more important is the Roccoco altarpiece with the image of Nossa Senhora da Piedade as the chapel has become an integral part of the pilgrimage.
And since the 16th century, annual festivities have been held to honour Nossa Senhora da Piedade or Mãe Soberana (Sovereign Mother). The yearly event, Festa de Mãe Soberana, includes a procession in honour of the town’s patron saint. The annual event is held in late April.
Walk into the dome chapel and see the voluminous church. And don’t miss the panoramic view of Loulé. You can see the whole town and the Gypsy Market from above, which is the next stop for this Loulé itinerary.

Address: R. Padre José António Nobre Duarte, Loulé | Hours: 9:00 am to 12:30 pm and 2:00 pm to 6:00 pm Tuesday to Sunday (closed Mondays)
14. Look at different knick-knacks at Gypsy Saturday Market
On Saturdays, many vendors set up temporary stalls at the Gypsy Market. There are literally rows and rows of vendors in a dusty parking lot selling many items like clothing, shoes, accessories, Portuguese souvenirs, etc. The quality is so so but everything is pretty cheap.
If buying knick-knacks is not your thing, this flea market has several food trucks. You can get snacks like donuts and bifana (sliced pork) sandwiches.

Address: Rua de Nossa Senhora da Piedade, Loulé | Hours: 8:00 am to 1:00 pm Saturdays only
15. See Duarte Pacheco’s Monument in Parque Municipal de Loulé
On the way back to the Old Town, take a short detour to Parque Municipal de Loulé and see Duarte Pacheco’s Monument.
The 17m tall stone monument was built in 1953 to commemorate Duarte Pacheco (1900 – 1943), a native Loulé citizen who was an electrical engineer by training and served several public positions in different ministries.
The pillar has 18 panels in low relief and each panel represents his life’s work.

Address: Parque Municipal de Loulé, Av. 25 de Abril 33, Loulé
16. Shop around Loulé Old Town
I always put the shopping option at the end of the itinerary so you don’t have to carry all your purchases with you all day.
And in Loulé, you might just buy a few things because there are many boutique stores selling locally made products like ceramics, clothing and accessories, and so much more.
The majority of the stores are on Rua 5 de Outubro, around Mercado Municipal de Loulé and all the small streets in between.
These are some of the shops in Loulé that really caught my eye:
Teresa’s Pottery – a really cute shop selling Portuguese ceramics.
Nouri – the store has so many ceramic bowls, cups, plates, etc.
MartinaTeresa’s PotteryNouri and Custódio Mealha Silva
Other things to do in Loulé during the week
If you are going to Loulé during the week, you can take a guided tour 230m beneath the city of Loulé and see the rock formation in a salt mine during a 2-hour tour under the city.
It would be cool to include the salt mine tour on a Saturday but this tour is only available from Monday to Friday at 9:30 am, 11 am, 2:30 pm and 4:00 pm.
So if you are going on a weekday, you can first follow my itinerary and then join this salt mine tour in the afternoon.
Are you ready to take a half (or full) day trip to Loulé Portugal?
I hope so! I really enjoyed my time in Loulé. The town itself is so quaint and walkable. You can spend half a day to see everything or you can take your time and spend the entire day in this charming town.
Let me know in the comments if you have any questions about travelling to Loulé or anything else about the quaint little town.
Thank you for reading my Loulé Portugal post

The diagnosis of petrositis and the difficulties during the diagnostic of this patient
Petrous apex syndrome, also known as Gradenigo’s syndrome [1], is a group of symptoms caused by lesions of petrous apex located at the temporal region that damage the abducens nerve and the ocular branch of the trigeminal nerve. It is a very rare clinic disease and mostly is caused by otitis media, mastoiditis and petrous apex tumor [6]. There is only one layer of dura mater between the semilunar ganglion, abducens nerve of the trigeminal nerve and the petrological apex of the temporal bone. So when the middle ear mastoiditis, cholesteatoma and petrological tumor occur at the petrological apex, these two nerves can be attacked. Main clinical manifestations are: pain in the trigeminal nerve area, loss of corneal reflex, diplopia and strabismus caused by abductive nerve paralysis, sometimes symptoms of meningeal irritation and peripheral facial paralysis [6,7,8].This disease usually is not primary disease in clinical practice often secondary [6,7,8]. Not all the classic triad symptoms are present. However, sometimes due to the severity of the disease, it may be worse than the “triad” [4]. There are other symptoms such as: ipsilateral paralysis due to involvement of the 7th cranial nerve, vertigo due to damage of the 8th, 9th and 10th cranial nerve, tongue leaning, and sensorineural hearing loss due to involvement of the inner ear labyrinth [3]. In this case, the patient first developed ear discharge, earache, and headache. With further development of the disease, facial paralysis, vertigo and diplopia symptoms showed up. However, we still need to exclude neoplastic lesions in nasopharynx, intracranial, lymphoma, tuberculosis and cerebrovascular diseases. One of the difficulties during the diagnosis of this patient was that the “triad” symptom showed gradually, with vertigo, paralysis of glossopharyngeal nerve, trochlear nerve, oculomotor nerve of many pairs of cranial nerve.
The second difficulty in the diagnosis of this patient was atypical imaging changes. CT and MRI examinations play an important auxiliary role in the diagnosis of porosities [9, 10]. MRI showed long T1 and long T2 signals in the petrous apex, which were enhanced after reinforcement. T2WI showed equal, slightly higher, or lower signals, bone damage in the lesion area; and involvement of adjacent structures including Meckel cavity and cavernous sinus. And enhanced scans showed obvious enhancement (Fig. 1E, F). Petrositis is very rare case for clinical practice. So CT imagine shows strong characteristic, it can sometime be mis diagnosed as middle ear mastoiditis. In addition, the petrous apex is a complex area with multiple variations that can be bilaterally symmetric or asymmetric [11]. This makes CT judgment more challenging. The patient’s CT image in this case showed blurred mastoid process, superior tympanum, tympanum sinus and petrous apex, with no obvious bone damage at the petrous apex (Fig. 1A, B). Although headache was present at the first admission, but patient was not diagnosed with petrositis, only at patient 2nd admission, with his symptoms of facial paralysis and diplopia, then after retrospective radiography (Fig. 1) he was diagnosed with petrositis.
The third difficulty was the patient’s pathogenic bacteria. Pseudomonas aeruginosa is the most common pathogenic bacteria of petrositis, then viridans streptococci [12, 13], Fusobacterium [14] and anaerobic bacteria [15] are secondary. There are still some reports on tuberculous petrositis [16,17,18]. We performed relevant exam to exclude tuberculosis infection. After blood mNGS examination combined with late bacterial culture and Periodic Acid-Schiff (PAS)staining of pathological sections, candida was finally identified as the pathogenic bacteria. This is very rare. There is one fatal case of fungal otitis media complication by petrositis was reported by an author in 2021, [19]. With today’s widely usage of antibiotics, fungal infection as a pathogenic bacteria need to be considered. To identify the pathogenic bacteria, we also used blood mNGS technology. This technology involves all the nucleic acid sequence in the specimen, which can cover all testing bacteria, mycoplasma, chlamydia, rickettsia, helix, fungi, viruses, parasites etc. [20]. One of the advantages of this technology is to detect culture-negative bacteria/fungal pathogens [21]. In this case, the secretion culture of the patient was negative for several times at the beginning, but by using this technology, it provided good reference (Fig. 2G). Through this case it demonstrates that Hematoxylin–eosin (HE) staining of pathological sections was not sensitive to the diagnosis of fungal infection (Fig. 2A), but PAS staining was sensitive to it (Fig. 2B). In this case, the routine blood leukocytes, c-reactive protein and amyloid A were higher than normal during entire illness (Fig. 3A–C). This misleads the diagnosis of fungal infection. The patients may have both fungal and bacterial infection coexistence. But at the end, patient was treated with antifungal drugs, sometime fungal infections can also lead to an increase in inflammatory indicators such as white blood cells level.
Another difficulty in the diagnosis of this patient was that his cranial magnetic resonance (MRV). It showed superior sagittal sinus thrombosis (Fig. 1I). The direct signs of head MRV in diagnosing superior sagittal sinus thrombosis are: losing of high blood flow signals in normal cerebral veins (sinuses) or sign of blurred and irregular low blood flow. This sign is not affected by temporal changes in the thrombus signal. This type of thrombosis can also lead to persistent and severe headache, but the patient had no typical thrombosis symptoms such as no unconscious disorder, no limb movement disorder, no intracranial hypertension (severe nausea and vomiting), no papilledema. His headache has not been reduced by simple antithrombotic therapy. Otitis media can lead to the thrombosis of sigmoid sinus and superior sagittal sinus. It can be caused by infection directly destroying bone and entering venous sinus, then resulting mural thrombosis, or by indirectly infection to the vein of the mastoid process, resulting local micro abscesses of blood vessels and infectious thrombosis [22]. In 2015, there were also reports of petrotitis complicated with cavernous sinus thrombosis and meningitis [23]. Theoretically, the patient’s left otitis media should have left sigmoid sinus thrombosis first. But he had superior sagittal sinus thrombosis instead of thrombosis (Fig. 1J) so that why we considered petrotits.
Treatment of petrotitis: surgery is risky and drug therapy is feasible
The difficulty in the treatment of this case was the options between surgery and conservative treatment, The drug therapy has been adjusted for several times. The ideal treatment of petrotitis is controversial and often depends on the severity of the clinical presence [24]. Before the invention of antibiotics, surgery was the main treatment for petrotitis. As there are two major controversies about the pathologic process of petrotitis, so different surgical approaches are suggested. Some experts believe that the petrous apex is independent of the mastoid area without small chambers of gasification, so the petrotitis is caused by local osteomyelitis [25]. The venous plexus of the internal carotid artery at petrous apex (hematogenous spread), is an important route of dissemination of inflammation. Therefore, the posterior labyrinth approach and temporal bone air-room resection and middle cranial fossa approach for abscess drainage are recommended [25]. Other experts believe that petrotitis like other gasified mastoid areas, is a kind of air-room fusion inflammation. and the air chambers around the internal carotid artery are an important route of inflammation dissemination, therefore, petrous apex resection is recommended [25]. In some recent reports, some surgeons combined transmastoid and middle cranial fossa [26]. Regardless which surgical approach, it is traumatic and high risk. Whether further reoperation should be performed for this patient who had undergone radical mastoidectomy? After analyzing the situation, we decided to use conservative drug treatment. When the patient was admitted to hospital for the first time, we had used the ceftriaxone and metronidazole for a month. And his ear secretions bacteria culture was negative. His headache was not released after using ceftazidime and levofloxacin according to experience. Patient condition was aggravated accompanied by peripheral facial paralysis and diplopia. We Adjusted antibiotics and upgraded to vancomycin combined with fluconazole, but we didn’t see any improvement. Finally, antifungal drug therapy was changed to cure to patient.
Recently, more authors have recommended the conservative treatment for non-surgical intervention and intravenous antibiotic therapy [8, 27, 28]. Plodpai et al. [29] reported a case of Gradenigo’s syndrome as secondary to chronic otitis media with previous radical mastoidectomy. This case was successfully treated by intravenous antibiotics and topical antibiotic ear drops. Some authors [30] summarized the diagnosis and treatment of petrotitis in the past 40 years and believed that antibiotics are still the main method for treatment. And Surgery is only performed for non-responding antibiotic cases. Most authors recommended to use cephalosporin antibiotics and metronidazole with or without vancomycin [30, 31]. Empirical intravenous antibiotics includes common drugs for bacterial mastoiditis (staphylococcus aureus, streptococcus pneumoniae, streptococcus pyogenes, and pseudomonas aeruginosa or anaerobic bacteria [32, 33]. In our case, ceftriaxone and metronidazole were used before admission to our hospital, then levofloxacin, ceftazidime, piperacillin, sulbactam and vancomycin were used after admission. Theoretically, if it was bacterial petrotitis, all these medications should be effective. But our case was caused by fluconzol-resistant fungus. There is no doubt that petrotitis caused by infection is equivalent to osteomyelitis, which requires intensive and prolonged antibiotic treatment to avoid recurrence [8].
Causes of fungal infection, characteristics of yeast in this case and treatment of fungal infection (emergence of drug-resistant bacteria)
In this case, the bacteria caused the pathogenic infection is Candida, we recalled that the patient had long term chronic otitis media and repeatedly used ofloxacin ear drops then he started to have ear pain and ipsilateral headache. Two combination broad-spectrum antibiotics had been used for more than 1 month before admission, but the condition didn’t get improved and became even worse. Thus, a fungal infection should be considered. However, the initial culture of ear secretions was negative, so our diagnosis and medication were misled by the result.
Candida is a kind of deep infection fungi, there are many species, the pathogenic candida includes: C. albicans, C. tropicalis and C. parapsilosis etc. [34]. Bacteria can secrete adenosine to block neutrophils then produce and release oxygen free radicals; Aspartic protease can be produced to degrade extracellular matrix and cause tissue damage. The common drugs for treatment are fluconazole, ketoconazole, amphotericin B and itraconazole [35, 36]. And fluconazole is the first choice for the clinical treatment of candida infection. It achieves bactericidal effect by affecting the biosynthesis of ergosterol in the fungal cell membrane and changing the permeability of the cell membrane. So it is considered as first option for treatment of deep fungal infection [37]. In this case, however, the patient was infected with a drug-resistant strain that was ineffective for fluconazole intravenous infusion.
With the extensive usage of antifungal drugs in the clinical practice, the composition ratio of variants candida bacteria changes, and candida bacteria’s drug resistance increases. According to some reports: for different type of candida bacteria, their sensitivity varies greatly with anti-fungal drugs [38]. Current data shows that both Candida tropicalis and Candida krusei have high resistance to fluconazole, and this can be natural resistance. Therefore, fluconazole should be used with super caution. We must analyze the candida identification and drug sensitivity test results to guide clinical drug use [39]. In recent years, there are more cases related to infection caused by Candida tropicalis and Candida tropicalis’s resistant to fluconazole [40,41,42]. The main mechanism of drug resistance is the mutation of drug target enzyme gene ERG11 [43]. As target enzyme encode is changed by ERG11, so efflux pump genes on cell membrane overreacts and biofilm is formed, etc. [44,45,46,47]. The ultimate mechanism is that fungal cells mutate and create bypass pathways. To prevent cell membrane changes and accumulation of toxic products, an alternative pathway is created to avoid interruption by azole drugs, and this allows fungi to maintain cell membrane function [48]. Amphotericin B is sensitive to several common candida, but it has a large side effect on human, so it is limited on clinic practice. Generally, reduced dosage of amphotericin B is combined with azole drugs to reduce side effects [45]. In this case, amphotericin B combined with fluorocytosine was used to treat the patient and finally achieve the healing effect.
Research on the treatment of drug-resistant fungi is currently a hot topic, and one study found that P-glycoprotein (P-glycoprotein)’s efflux inhibitors P22CP and P34CP can reduce the FLC (fluconazole) values of multidrug-resistant Candida strains. This suggests that efflux activity contributes to the overall resistance of microbial strains [49]. Efflux- inhibitors, EIs) inhibiting the efflux pump can enhance the clinical effectiveness of antibiotics as their substrates [49]. So the search for efflux pump inhibitors (EIs) is the direction of research for the treatment of MDR-causing bacteria [50]. There are also studies on plant-derived products and essential oils to treat multidrug-resistant Candida, such as Ruta graveolens essential oil (REO) [51], oil macerate of Helichrysum microphyllum Cambess [52], thyme oil ( Thymus vulgaris essential oil) [53], etc., Good results have been achieved by either using alone or combining with antifungal drugs. Chitosan (chitosan) has also been found to have a significant synergistic antifungal effect in combination with fluconazole (fluconazole) [54]. It is effective against both Candida species and their resistant strains.
The first difficulty in the diagnosis and treatment of this patient was that the typical symptoms only appeared gradually slowly, and even beyond the “triad of symptoms”, it also showed multiple pairs of cranial nerve involvement; The second difficulty was the initial imaging presentation was atypical and cerebrovascular magnetic resonance showed symptoms thrombosis and stenosis, led to different conclusion. The third difficulty was that the treatment with a variety of advanced antibiotics and conventional antifungal drugs were ineffective. In conclusion, when otitis media is combined with persistent and severe headache, it is important to consider the possibility of Petrositis, even the imagine analysis doesn’t obviously indicates. Conservative drug treatment is a feasible choice. Broad-spectrum with high-efficient antibiotics should be added for treatment while bacterial culture of secretions is performed. Fungal infection or even drug-resistant fungal infection should be considered, when treatment was not effective, and medication should be adjusted in time. Blood NGS tests can also provide good reference.

Writing efficient code and choosing the right algorithms can significantly improve the overall performance of your application. In this section, we’ll explore some best practices for optimizing code and algorithms to achieve peak performance.
Efficient Code Writing Practices
Writing efficient code is essential for optimizing application performance. Here are some practices you can follow:
Eliminate unnecessary computations: Identify any redundant calculations in your code and eliminate them. This can help reduce the workload on your application and improve its responsiveness.
Minimize memory usage: Be mindful of memory usage and avoid unnecessary object creation or memory allocations. Use data structures that are efficient in terms of memory usage, such as arrays instead of linked lists.
Avoid excessive looping: Excessive looping can lead to performance bottlenecks. Whenever possible, try to optimize your loops by reducing the number of iterations or using more efficient looping constructs.
Use appropriate data types: Choosing the right data types can have a significant impact on performance. Use data types that are appropriate for the values you are working with. For example, use integers instead of floating-point numbers when not required.
Optimize I/O operations: Minimize I/O operations as they are typically slow compared to other operations. Instead of making multiple I/O calls, try to combine them or perform batch operations whenever possible.
Choosing the Right Data Structures and Algorithms
The choice of data structures and algorithms can greatly impact the performance of your application. Consider the following:
Data structures: Use data structures that provide efficient operations for your specific use case. For example:
Arrays: Ideal for random access and efficient memory usage.

“The best code is no code at all.” – Jeff Atwood
Keep in mind that optimization should not be prioritized over code readability and maintainability. Write clean, modular code that is easy to understand and requires minimal modifications. Regularly review and refactor your code to ensure optimal performance.
In the next section, we’ll explore another crucial aspect of optimizing application performance: improving resource management.
Improving Resource Management
As an application developer, optimizing resource management is crucial to ensure your application performs at its peak. Efficiently managing resources such as memory and CPU usage can greatly impact the overall performance of your application. In this section, we will explore some strategies to improve resource management and boost application performance.
Effective Memory Management
Managing memory efficiently is essential to avoid memory leaks, reduce memory consumption, and increase the overall performance of your application. Here are some best practices to consider:
Minimize object creation: Creating too many objects can lead to excessive memory usage. Consider reusing objects or using object pooling techniques to minimize memory allocation.
Avoid memory leaks: Memory leaks occur when objects are no longer used but still hold references. Make sure to release resources and de-reference objects that are no longer needed.
Use efficient data structures: Choose data structures that are optimized for memory usage. For example, consider using arrays instead of lists when the size is known in advance to reduce memory overhead.
Implement garbage collection: Garbage collection helps reclaim memory occupied by objects that are no longer in use. Optimize garbage collection settings to minimize its impact on the application’s performance.
Optimizing CPU Usage
Efficiently utilizing the CPU can significantly improve application performance. Here are some strategies to optimize CPU usage:
Reduce unnecessary computations: Identify and eliminate any redundant or unnecessary computations. This can be achieved through code refactoring and algorithm optimization.
Multithreading: Utilize multiple threads to distribute workload across CPU cores. This can improve performance, especially for CPU-intensive tasks.
Asynchronous programming: Use asynchronous programming techniques to avoid blocking the CPU while waiting for I/O operations to complete.
Batch processing: Group similar tasks to minimize CPU context switching and overhead.
By implementing these strategies, you can optimize resource management and maximize the performance of your application. Remember, efficient memory management and CPU usage are key factors in achieving peak application performance.
Utilizing Caching Techniques
Caching is a powerful technique that can significantly improve application performance by reducing the time it takes to retrieve and process data. By storing frequently accessed or computationally expensive data in a cache, subsequent requests for that data can be served faster, resulting in improved response times and overall user experience.
Implementing Data Caching
There are several ways to implement data caching in your application. Let’s explore some popular caching techniques:
Page-level Caching: This technique involves caching entire web pages or portions of pages to avoid executing expensive database queries or complex calculations on each request. Page-level caching is particularly useful for static or semi-static content that doesn’t change frequently.
Fragment-level Caching: Instead of caching entire pages, fragment-level caching focuses on caching specific sections or components of a page. This can be useful for dynamic content that is expensive to generate, but only a portion of it changes frequently.
Data Caching: Data caching involves caching query results or individual objects to avoid repetitive database queries. By caching frequently accessed data, you can reduce latency and improve the overall performance of your application.
Object Caching: In object caching, you store whole objects in the cache, allowing you to retrieve and reuse them without recreating or re-fetching data from the source. This can be especially useful for complex objects that require significant processing time to generate.
Caching Strategies
Once you’ve decided to implement caching in your application, it’s important to determine the right caching strategy for your specific use case. Here are some common caching strategies:
Time-based Expiration: This strategy involves setting a specific duration for which the cached data is considered valid. After this time has elapsed, the data is considered stale, and a fresh copy needs to be fetched. This approach is useful when the data doesn’t change frequently and can be served from the cache for a specific period.
Invalidation-based Expiration: In this strategy, the cached data is invalidated or cleared whenever the underlying data changes. This ensures that the cache always contains the most up-to-date information. This approach is suitable when the data changes frequently, and it’s crucial to have the latest version.
Lazy Loading: Lazy loading involves caching data only when it’s requested for the first time. Subsequent requests for the same data can be served from the cache. This strategy is useful when the initial request for the data is expensive, and subsequent requests can benefit from the cache.
Multi-tiered Caching: In this strategy, multiple levels of caching are used, with each level having different capacities and retrieval speeds. The most frequently accessed data is stored in the fastest and smallest cache, while less frequently accessed data is stored in larger, but slower caches. This approach optimizes performance by minimizing cache misses.
By implementing the right caching techniques and strategies in your application, you can significantly improve its performance and provide a better user experience. However, it’s important to analyze your application’s specific requirements and choose caching strategies that align with those requirements. Remember to periodically monitor and fine-tune your caching implementation to ensure optimal performance.
Scaling and Load Balancing
Scaling applications for high traffic and implementing load balancing are vital strategies to achieve peak application performance. As your application grows and attracts more users, it’s essential to ensure that it can handle the increased load efficiently. Let’s explore the key concepts and techniques related to scaling and load balancing.
Scaling Applications for High Traffic
When it comes to scaling applications, there are two main approaches: vertical scaling and horizontal scaling.
Vertical Scaling: This involves upgrading the server hardware, such as increasing CPU power, memory, or storage capacity. While vertical scaling can provide a temporary solution, it may become expensive and limit potential scalability in the long run.
Horizontal Scaling: This technique involves adding more servers to distribute the load and handle increased traffic. Horizontal scaling allows for better scalability and can be more cost-effective.
To scale your application effectively, consider the following strategies:
Distributed Architecture: Adopt a distributed architecture, such as a microservices-based structure, that allows you to divide the application into smaller, independent components. This approach facilitates horizontal scaling by allowing you to scale individual components independently.
Database Scalability: Optimize database performance and scalability by implementing techniques like sharding, replication, or using a distributed database system. These approaches can help distribute the database load across multiple servers and improve application performance.
Server Load Balancing: Utilize a load balancer to distribute incoming traffic evenly across multiple servers. Load balancing ensures that each server handles an equal share of the workload, minimizing the risk of server overload and maximizing resource utilization.
Distributing Workloads with Load Balancing
Load balancing is an essential technique to evenly distribute incoming traffic among multiple servers. Here are some popular load balancing strategies:
Round Robin: Requests are rotated sequentially across multiple servers. This approach is simple and effective when the servers have similar performance characteristics.
Weighted Round Robin: Servers are assigned different weights based on their capabilities. Servers with higher weights receive a larger portion of the incoming traffic. This strategy is useful when servers have varying performance capacities.
Least Connections: Incoming requests are forwarded to the server with the fewest active connections. This approach ensures that the workload is evenly distributed based on the current server load.
Session Affinity: Also known as sticky sessions, this technique routes a user’s requests to the same server throughout their session. It can be useful for applications that require maintaining session states, such as e-commerce platforms or user authentication systems.
Load balancing can be implemented using hardware load balancers or software-based load balancers. Some popular software load balancers include NGINX, HAProxy, and Apache HTTP Server with mod_proxy_balancer module.
Note: When implementing load balancing, it’s essential to monitor the health and performance of servers continuously. Use health checks to detect any failures or excessive load on servers and dynamically adjust the load balancing algorithm accordingly.
By implementing scaling and load balancing techniques, you can ensure that your application can handle high traffic volumes efficiently. Whether you choose vertical scaling, horizontal scaling, or a combination of both, remember to design your application in a way that allows for easy scalability in the future.
Monitoring and Profiling
As a developer, it’s essential to constantly monitor and profile your application’s performance to ensure it’s running smoothly and efficiently. Monitoring and profiling can help you identify bottlenecks, resource usage, and potential security vulnerabilities. In this section, we’ll explore the importance of real-time performance monitoring and the available profiling tools for application analysis.
Real-time Performance Monitoring
Real-time performance monitoring allows you to keep a close eye on your application’s performance and identify any potential issues or bottlenecks as they occur. By monitoring key metrics such as response times, CPU and memory usage, and network latency, you can quickly pinpoint areas that need optimization.
Some benefits of real-time performance monitoring include:
Early detection of issues: Real-time monitoring enables you to detect performance issues and errors immediately, allowing you to take proactive measures before they impact the user experience.
Optimization opportunities: By constantly monitoring your application, you can identify areas that can be optimized for better performance, such as slow database queries or high CPU usage.
Capacity planning: Real-time monitoring provides insights into resource usage, allowing you to plan for future capacity needs and scale your application accordingly.
